New Employee Onboarding Forms

Congratulations on your employment with Weldon City Schools! As part of the employment process, you will be scheduled to attend a New Employee Onboarding with Human Resources. 
  • Appropriate identification for completion of the federal Employment Eligibility Verification Form (I-9).
  • You will need to bring original forms of your identification documents (no photocopies or faxed copies)
  • An original Social Security card for payroll purposes (no copies)
  • Completed Health Certificate with TB results.
  • A voided check for direct deposit
  • Completed New Hire forms 
Prior to this session, please complete the following forms and bring them with you to the New Employee Orientation. Access each form by clicking on the appropriate link.
 
1. Personal Information Form
2. Employment Eligibility Form I-9
Complete “Section 1. Employee Information and Attestation.”
3. N. C. Tax Form (NC-4)
Complete the NC-4.  
4. Federal Tax Form (W-4)
Complete “Employee’s Withholding Allowance Certificate” (bottom section).
5. Health Examination Certificate
This form is to be completed by your physician. If you do not have it completed in time to bring it with you to the New Employee Orientation, you must return it to Human Resources within 30 days of your effective date of employment. 
6. Employee Record of Aggregate Service
List only employment you have had as an employee with the state of North Carolina, ending with your new position in this school district.
6.a. If you have been previously employed with another North Carolina state agency, please complete the top portion of this form and send it to the last state agency with which you were employed. Please bring a duplicate copy to New Employee Orientation so that WCS will have a record of what agency you have requested verification of prior employment.
7. Direct Deposit Form
Attach a voided check for the account to which you wish your monthly check to be deposited.  If you don't have checks, please have your institution print on their letterhead the following items.

8. Employee Acceptable Use Policy/Form
Complete this form and indicate if you need training or a workshop related to using email or the internet.

View this information to assist you in your insurance coverage decision-making prior to the New Employee Orientation you will be attending.
11. Voluntary Equal Employment Opportunity Identification Form

In addition to completing the forms listed above,
please bring the following items with you to the session:
1. Two forms of identification, e.g., social security card and driver’s license. For other acceptable items, see the Employment Eligibility Form I-9.
2. Selective Service Registration Card (males 26 years of age or younger only). If you cannot locate your card, call the Selective Service Office (1-847-688-6888) and request a duplicate.
Certified/Licensed Employees 
In addition to these forms, you will need to register and complete an application for a North Carolina Educator License at NCPDI Online Licensure. Salaries for licensed employees are determined by the North Carolina Department of Public Instruction (NCDPI). Licensed employees are paid at the beginning level of teacher pay until the teaching licenses are processed. Please reference the North Carolina Educator Compensation website.
Once NCDPI issues a license that gives credit for years of experience or advanced degrees, the employee’s salary can be adjusted retroactive to the date established by NCDPI.
Therefore, it is critical to obtain the following documents and submit to Human Resources:

Classified Employees:
In addition to the New Employee forms listed above, please ensure you submit to Human Resources a certified copy (if possible) of your High School Diploma or GED. Salaries are determined by Weldon City Schools and the NCDPI Classified Pay Scale.
If you are offered employment as a Teacher Assistant or any other position which requires an Associates degree or 48 hours of approved/relevant coursework, please submit an official transcript to Human Resources prior to your start date of employment.
